We are always accepting new patients at Oak Family Dentistry. There are several ways to contact us in order to schedule an appointment. Feel free to fill out this appointment request and we will be in contact with you as soon as we receive the information or call one of our staff members and they will be glad to help you with any questions you may have.
You may also cut down your appointment time by printing and filling out the following forms and bringing them to your first appointment.
Our staff will be more than happy to assist you in filling these forms out when you come in.

Dental Emergency
Toothaches and Facial pain are not something that should be ignored. We urge you to give us a call and schedule an emergency appointment as soon as possible. Chipped and broken teeth can lead to discomfort and if not addressed in a timely manner can lead to further problems. We understand you want issues such as these fixed as soon as possible.
